Instructor Forms

The following are some of the forms the ICAA requires the instructor to fill-out and submit.

Instructor Evaluation Form

Extremely important in ensuring that the material being taught in the seminar is up to date, and relevant. Our instructors are the experts on the subject, so we expect them to have a major role in ensuring the accuracy, in both content and delivery of the material.

Expense Claim Form

Filled out and submitted by the instructor to the ICAA, for any agreed upon expense that the instructor has accrued as a result of teaching seminar. Please also see travel reimbursement guidelines for clarification on what can be claimed, and when to do so.
